Vatican City Country Code is used when dialing to Vatican City from outside Vatican City. Vatican City Country Code is however not used when dialing within Vatican City.
To call Vatican City from outside Vatican City, you have to dial the your own country's International Direct Dialing prefix (IDD), Vatican City Country Code, City/ Area Code, and then the subscriber telephone number as:

Do not use Vatican City National Direct Dialing prefix when dialing from outside Vatican City into Vatican City. Vatican City NDD is only used as long distance dialing within the country.
